Data Consent Form for Attenders
- | We need to hold information about our members and attenders, and will protect such information from unauthorised use. The Data Consent forms are a way to collect basic information - names, addresses, phone numbers, email addresses - with options for you tell us that some or all of your details are not to be included in published address lists or to be passed to the Yearly Meeting database or to other Quaker bodies.
| - | Information on non-member attenders, and the non-member spouses/partners of members, may be held and used only with their consent. Information on children not in membership may be held and used only with the consent of their parent or guardian.

Please give or send your completed Data Consent form, or your notification of any change, to your Local Meeting Clerk, or to the Area Meeting Clerk (amclerk@hhq.org.uk)
